The Continuum of Care (CoC) Program is designed to promote communitywide commitment to the goal of ending homelessness; provide funding for efforts by nonprofit providers, and State and local governments to quickly rehouse homeless individuals and families while minimizing the trauma and dislocation caused to homeless individuals, families, and communities by homelessness; promote access to and effect utilization of mainstream programs by homeless individuals and families; and optimize self-sufficiency among individuals and families experiencing homelessness (Department of Housing and Urban Development).

JoLyn spent the first 20 years of her career working with foster children and the TAY population in the child welfare system. When foster care was extended to age 21 in 2012, JoLyn became involved with Transitional Age Youth (TAY) housing programs, running two programs in San Joaquin County. JoLyn then became Executive Director of a sober living family shelter in Solano County. During that time JoLyn also ran the agency’s transitional apartment program using vouchers to subsidize rent and providing case management services to assist parents with maintaining sobriety and housing. JoLyn then went on to become CEO of the largest low barrier shelter in Northern California which is located in San Joaquin county. The shelter served over 400 individuals and families each night. The shelter’s programs included families, single men, Veterans, Housing Opportunities for People with AIDS/HIV (HOPWA), and PSH. During JoLyn’s time at the shelter, the first low barrier single women’s program in San Joaquin County was opened to serve single women experiencing addiction and/or untreated behavioral health diagnoses.
The Continuums of Care for Placer County and Nevada County are standing advisory committees to HRCS. and brings people together to prevent and end homelessness in Placer County and Nevada County.
The meetings of CA-531 are held on the third Thursday of each month 1:30-3:30 p.m. and are being held by virtual teleconference until further notice.
The Board of Directors meets virtually on the the 4th Thursday of every month
